The Transition Project

The Chapel Hill-Carrboro Transition Project is one of thirty-two federally funded programs nationwide which are studying the benefits of providing family support and developmentally appropriate programs for Head Start and other at-risk children as they enter kindergarten and progress through elementary school.

Seawell, Estes Hill, and Frank Porter Graham elementary schools in Chapel Hill, North Carolina, were randomly selected to receive transition services. An evaluation of the effectiveness of these support services is being conducted by the Frank Porter Graham Center at the University of North Carolina. The data collected will be complied with the research data from the other national sites.

Transition Services

  • Transition teams composed of an Education Coordinator and Family Service Coordinator located at the Transition school work with children, families, and school personnel to facilitate using the home as a place of learning and reinforcement, and to provide support to the families in the areas of health, nutrition, literacy, and social services.
  • Training and information for public school, Head Start, and other preschool personnel serving young or at-risk children in areas such as developmentally appropriate curricula, multicultural awareness, parental involvement, family empowerment, and conflict resolution strategies.
  • Outreach to help establish transition procedures for the community, as well as to facilitate a delivery system of services more accessible to all families.

Transition Staff

  • assist families' access to medical care
  • help families secure social services
  • encourage communication between parent and classroom teacher
  • provide educational enrichment activities in the home and school
  • provide individualized and group parent programs
  • encourage parent involvement with the child's education
  • assist families in securing adequate housing
  • help families develop preventive health routines such as yearly well check-ups, immunizations for children, and dental check-ups
